Guidelines for filling up the form:

• Please enter core personal details to go to the Application Form Page.
• If an applicant is minor, kindly give Name, PAN and KYC details of Guardian.
• KYC is mandatory for all the the applicants.
• Fields highlighted in red are mandatory.
• Attach the required documents and send to the nearest AMC/Karvy Investor Service Centres. Please click here  to view the addresses of Investor Service Centres.

Supporting documents to be Attached along with application form:

• Cheque/DD should be drawn in favour of scheme name. Kindly mention the applicant name and application no. on reverse of Cheque/DD . Please note, outstation cheques will not be accepted.
• Self attested Copy of PAN Card for all the applicants.
• Self attested KYC letter for all applicants. If any applicant is not KYC compliant, kindly fill the KYC form, attach required documents and submit to the nearest AMC / Karvy Investor Service Centre.
